Service & Support Administrator (Case Manager)This position requires an Associates or Bachelor’s Degree, preferably in human services related field. The Service & Support Administrator (SSA) assesses individual needs for services, coordinate Individual Service Plan (ISP) conferences, advocates for enrollee and their family, provides training and supports that will lead to accomplishment of enrollee's personal goals and objectives, Identifies and accesses resources on behalf of the enrollee and their family, maintains documentation and records, and other duties as written in the full job description.

Community Living SpecialistThis position requires a High School Diploma or GED. This is specialized work promoting connections, inclusion and growth opportunities to foster independence through community and recreation activities. Activities may take place in a variety of locations depending on the individual’s abilities, needs and desires. This position works directly with individuals served by the Board of DD and will primarily provide opportunities for recreation and activities on evenings and some weekends.
